  • As a foster for New Leash On LIfe Rescue, you agree to take your foster dog to New Leash on Life Rescue authorized Veterinarian. You Must contact Edna Roberts prior to making any appointments at our veterinarian. If there are immediate health concerns, please contact New Leash On Life Rescue as soon as possible. Please do not make appointments with our vet, your personal vet or any other vet without having prior authorization.   • Accident Waiver, Release of Liability and Terms of Foster Agreement

    FOSTER CONTRACT/AGREEMENT
  • I will be working as a volunteer for New Leash On Life Rescue. I understand that I will have direct physical contact with the animals at a shelter, adoption events, or in my home as a foster parent and that contact carries inherent risks of injury and damage to me personally; I will obey any instructions given to me by official staff of New Leash On Life Rescue and will carefully read all messages pertaining to each animal with which I have contact. I acknowledge that my presence at the animal shelter or in my home as a foster parent carries the potential for accidental injury and/or property loss. The risks include, but are not limited to, those caused by the condition of the facility, injury or illness as the result of general or direct contact with animals, the actions of other people and/or vehicles. I hereby assume all risks of acting as a volunteer. I acknowledge that this Accident Waiver and Release of Liability form will be used by New Leash On Life Rescue, its employees, volunteers, representatives, agents, and others working on behalf of New Leash On Life Rescue and that it will govern my actions and responsibilities. In consideration of my participation as a volunteer, I hereby take actions for myself, my executors, administrators, heirs, next of kin, successors and assigns to:

    • Waive, release and discharge from any and all liability for my death, disability, personal injury, property damage, property theft, or actions of any kinds which may hereafter accrue to me, including as to my traveling to and from this activity, the following entities or persons: New Leash On Life Rescue, its employees, Board of Directors, volunteers, representatives, agents, veterinarians and others working or acting on behalf of New Leash On Life Rescue, and
    • To the fullest extent permitted by law, hold harmless the entities or persons mentioned above from any and all liabilities or claims made by other individuals or entities as a result of or relating to my participation as a volunteer for an indeterminate amount of time or until I provide written notification to New Leash On Life Rescue that I resign my position as a foster volunteer with the organization.

    I hereby consent to receive medical treatment of which may be deemed advisable in the event of injury, accident, and/or illness during my participation as a volunteer. I further agree that I will defend, indemnify, and hold harmless New Leash On Life Rescue, its officers, directors, members and agents, employees, volunteers, and veterinarians or any of them against all claims, demands and causes of action or other proceeding brought by or prosecuted for my benefit contrary to this release extended to all claims of every kind and nature whatsoever whether known or unknown and expressly waive any benefits. I hereby certify that I have read this document and understand and agree to its content.

    Foster Terms
    As A New Leash On Life Rescue Foster You Agree to:

    House and care for the animal in an appropriate manner, reinforcing manners, working with problem areas, feeding appropriately and housing indoors only.

    You are not to give the animal to anyone at any time that has not completed a foster or adoption application and has been approved by a New Leash On Life Rescue representative.

    You will care for the animal until it finds its forever home, but if it doesn’t work out you must contact New Leash On Life Rescue and allow a reasonable time frame (up to 2 weeks) to find another foster home for the animal. This is crucial to all involved.

    When your foster animal is due for vetting, you must be able to bring them to one of our approved Vet clinics. You are usually able to make your own vet appointments at your convenience as long as it’s within a week’s time frame that the animal is due for shots, sterilization, etc. We will notify you of any vetting needed.

    Any animal that is brought outside is to be on leash at all times unless in fenced in area. You are 100% financially responsible for any preventable medical expenses while the foster pet is in your care. This includes any decisions you make to leave the pet at a friend’s house for the day, or while on vacation. If anything happens to the foster pet or person while being watched out of your care, you will also be 100% financially responsible. Please use common sense and if you have any questions on what is acceptable or not contact a New Leash On Life Rescue representative.

    If you plan a vacation or a weekend getaway, you are responsible finding someone within the rescue to watch your animal, a family member or trusted friend. Summer weekends are very busy for everyone so please plan accordingly and know that when you commit to fostering, you commit to the responsibilities as if the foster pet were your own. If you leave the pet with someone, please give them all of our contact information while you are gone, and please notify us of the arrangements being made prior to your departure.

    If you feel your foster pet(s) need(s) immediate medical attention, you MUST get prior approval from New Leash On Life Rescue. We will not reimburse if you choose to seek medical attention without first contacting us. We have relationships with numerous clinics and will most likely be able to refer you to one of them. You will administer any medications as directed from our veterinarian.

    You understand that the animals that come into the rescue may not have yet been vetted, and New Leash On Life Rescue is in no way liable for any animals other than the foster pet if they become sick.

    If you decide that you want to participate in our “Foster to Adopt” program, you can plan on adopting the animal but can’t initially adopt until the animal is completely vetted through New Leash On Life Rescue. We give you the first 14 days to decide if this specific pet is right for you and your family. After that, if you are unsure or know that it’s not the right fit, we will continue the search for their forever home. In the meantime you agree to foster the animal until we can find their new forever home. We know you are anxious at finding the perfect pet for your family, but just remember that fostering can take anywhere from 3 weeks to months. There is no simple way to determine how long it will take any one animal to find a new home. Either way, fostering or adopting, you are saving an animal and giving them a whole new quality of life.

    You understand that fostering is at your own risk and you hereby forever release, discharge and agree to hold harmless and indemnify New Leash On Life Rescue from all claims, demands, actions, causes of action or liability of any kind whatsoever arising as a result of doing volunteer work with New Leash On Life Rescue.

    Your participation with New Leash On Life Rescue is strictly on a voluntary basis, therefore no insurance against bodily harm is provided for me.

    You agree to cooperate with New Leash On Life Rescue and its volunteers to coordinate arrangements for adoption days, vet appointments, and other necessary appointments.

    I HAVE READ THIS AGREEMENT CAREFULLY AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS AN INDEMNITY, WAIVER & RELEASE OF LIABILITY AND A FOSTER CONTRACT BETWEEN NEW LEASH ON LIFE RESCUE, AND YOU SIGN IT OF YOUR OWN FREE WILL.
